Job Description
Continuing Care LPN Arbours - Full Time Days Position Summary: Executes duties under the direction and supervision of the RN Supervisor, administers medications and completes treatment procedures during assigned work time, and participates in admission process and in the development of a comprehensive plan of care for each resident. The Licensed Practical Nurse will incorporate the Carle values of Teamwork, Accountability, Acknowledgement, and Empowerment in all daily duties. ESSENTIAL JOB FUNCTIONS ** Provides direct patient care as assigned to residents and/or to be determined through accurate assessment of patient's needs/condition. ** Administers and documents medications and treatments in accordance with facility policies and procedures. ** Utilizes principles of rehabilitative and restorative nursing in the provision of patient care. Documents response to care on a continuing basis. ** Contributes to, utilizes, and communicates the residents MDS and care plans in accordance with facility policies and procedures. Attends Care Plan meetings/Medicare weekly meetings under the direction of the Resident Assessment Coordinator. ** Utilizes appropriate infection control techniques in performance of resident care; supervises and enforces compliance from ancillary personnel. ** Provides services and documents such services on appropriate forms per facility policies and procedures. ** Reports condition changes and resident’s needs to physicians, families, nursing management, and on-coming shift personnel. ** Maintains internal verbal and written communications with other staff members, allied departments, and nursing management. ** Complies with the process and time frames for completion of resident assessments, MDS, Care Plans, RAP modules/summaries and updates/revises these as resident condition dictates. ** Obtains, transcribes, communicates and carries out physician orders. ** Maintains knowledge and demonstrates professional principles in the delivery of skilled nursing procedures. ** Monitors the performance of and provides direction to orientees assigned personnel and Certified Nursing Assistants assigned to their unit. ** Promotes cost containment through efficient use of equipment, supplies, and time. ** Maintains a safe, accommodating and congenial environment for residents, visitors, and staff. ** Maintains exemplary customer relationships; is responsive to needs; utilizes resource staff as needed to assure patient/family satisfaction. ** Acquires/maintains knowledge of and adheres to the requirements of licensing/regulatory entities. Qualifications: Education Requirements: Licensed Practical Nursing Degree. Required Licensure/Certifications: Current license to perform practical nursing duties in the State of Illinois or letter of verification of the impending licensure. Experience: Acute care, restorative care, subacute care, long term care. Other Skills: Good written and verbal communication skills. Must be able to work without direct supervision. Must be able to follow oral and written instructions. Possesses counseling, strong interpersonal skills.
