Job Description
Home Services Field RN (FT) - Department: Hospice Skilled Nursing - Category: Nursing - Full Time Days Position Summary: The staff RN is a professional caregiver who is a member of the interdisciplinary team. Their responsibilities include coordination and provision of direct and indirect patient care using the nursing process to meet the physical, psychosocial, environmental and spiritual needs of Carle Home Services specific patient populations and families throughout the geographical area. ESSENTIAL JOB FUNCTIONS: ** Performs assessment and data collection of specific patient populations, family, and caregivers in an ongoing and systematic manner, focusing on physiological, psychological and cognitive status. ** Formulates a goal-directed plan of care which is prioritized and based on determined patient problems and patient outcomes. ** Implements care in a knowledgeable, skillful, consistent and continuous manner through utilization of and adherence to pre-determined standards which define the structure, process, and desired patient outcomes of the nursing process. ** Evaluates effectiveness of plan of care and care given by self and health care team members and the contribution of systems, environment, and technology in progressing patient, families, and caregivers toward desired outcomes ** Educates specific patient populations, families, and/or other care providers on care techniques. ** Communicates and coordinates exchange of information with other health care team members to ensure patient care outcomes. ** Completes documentation as per policy and procedure in order to maintain a thorough and accurate clinical record for each patient. ** Demonstrates knowledge of and the correct administration of medications and the correct performance of procedures for specific patient populations. ** Demonstrates a growing and applied knowledge base in areas of standards of care, patient care resources and references, protocols, pertinent drugs, significant lab values, and procedures. ** Assesses, identifies, and reports safety issues for patients and staff. ** Demonstrates awareness of legal issues in all aspects of patient care and strives to manage situations in a reduced-risk manner. ** Performs efficiently in emergency situations following established protocols. ** Provides care and practices in a cost-effective and cost-contained manner. ** Demonstrates effective and appropriate communication methods and skills and uses lines of authority appropriately. ** Participates actively in self and staff development activities for the department and home services personnel. ** Participates in the rotation of night, weekend, and holiday on-call coverage and responds appropriately to call situations. ** Demonstrates time management and critical thinking skills necessary to work autonomously and prioritize and organize workload. ** Demonstrates the use of the Carle Foundation and Home Services Customer Service expectations in all interactions with internal and external customers. ** Remains flexible in staffing patterns and participates in the resolution of staffing conflicts. ** Identifies problems in the delivery of health care and participates in developing a plan of change for quality improvement. ** For Infusion RNs: Performs Peripherally Inserted Central Catheter and/or Midline Catheter insertions for specific patient populations following national standards and policy and procedures. ** For Infusion RNs: Participates with the pharmacist to enter and process new prescriptions and refills in the computer and to monitor, maintain, prepare, and pick orders for drugs and related supplies for specific patient populations. ** For Infusion RNs: Performs miscellaneous billing functions including patient charging through manual entry into the computer and generation of billing forms. Qualifications: Education Requirements: Associate Degree, Nursing. Education Preferences: Bachelor's Degree Preferred, nursing. Relevant work experience will be evaluated. Required Licensure/Certifications: Current license as a Registered Nurse in Illinois. Cardio-Pulmonary Resuscitation Certification For CIS nurses- Licensed as a pharmacy technician. Preferred Licensure/Certifications: For CIS nurses- Certified Registered nurse in Intravenous Therapy (CRNI) and Peripherally Inserted Central Catheter and Midline catheter insertion certification. Experience: A minimum of one year of nursing experience.
